Downstream Oxygen Sensors Swapped From Bank To Bank
Code P0041 means the PCM has detected that the downstream oxygen sensor signals for Bank 1 and Bank 2 (Sensor 2 — post-catalyst) appear to be swapped. The downstream sensors monitor catalyst efficiency for each bank; if they are reading from the wrong banks, catalyst monitor data will be corrupted. The cause is virtually always physical sensor or connector swapping during a previous repair.

Reconnect the downstream sensor connectors to the matching sensors on each bank.

Clear codes and run catalyst monitors
After correcting sensor positions, clear DTCs and run the catalyst efficiency monitors to verify each bank reports correctly.
